HOW TO REQUEST RECORDS:
If you would like to request records maintained by the Consumer Protection Board (CPB) pursuant to the Freedom of Information Law (FOIL), Article 6 of the Public Officers Law, you must contact the CPB using one of the following methods. Certain requests will require you to provide additional, follow-up information. Please see details below:
ELECTRONIC REQUESTS - You may submit a FOIL request electronically. Electronic requests for records should be addressed to: webmaster@consumer.state.ny.us with subject line: FOIL Request.
BY REGULAR MAIL - Record requests may also be submitted by mail to FOIL Request, Records Access Officer, NYS Consumer Protection Board (CPB), 5 Empire State Plaza, Suite 2101, Albany, NY 12223.
BY FAX - You may fax your FOIL request to (518) 474-2474.

To assist CPB Staff in filling your request for CPB records, your request should include:
As many specific details as possible to identify your requested records including any file number assigned to the matter; and,
The names, dates and addresses involved in the matter.
